Community Connectors

The Practice has dedicated social prescribers (Connectors) who provide two sessions each week to see patients who are experiencing non-clinical problems or who have a clinical problem that is being exacerbated by societal factors. The Connector can assist with:

  • Housing issues
  • Debt/ Hardship/Welfare Rights
  • Difficulties in parenting
  • Common mental health problems
  • Relationship difficulties
  • Domestic abuse
  • Social isolation
  • Carers issues
  • Home adaptations/assistive technology
  • Health and exercise

This is not an exhaustive list.

The Connector provides support to access all the services provided by the Councils and their associated Help Hub partner agencies.
