Weight Management Self Referral

Apply for the free weight management support.

If you’d like to apply for the free support, you’ll need to fill in the referral form.

Measure your height and weight and have those measurements to hand when you fill in the form. You’ll need to provide these so we can calculate your BMI.

To be eligible for the free support you must be:

  • Aged 18 or above.
  • Have a BMI of 30 or above or, if you are Black, Asian or from another minority ethnic group, a BMI of 27.5 or above. The BMI is lower due to the higher risk of poor health.
  • A Norfolk resident, or be registered with a Norfolk GP practice.

You will not be eligible if:

  • You’re currently pregnant.
  • You have attended one of our funded weight management services in the past 12 months.
  • You have been a paying member of Slimming World or Your Health Norfolk (weight management sessions) in the past three months.

Man V Fat Football

Local, competitive football leagues for overweight men are being set up across Norfolk. Places are funded with their first season of 14 matches free of charge. Players also receive dietary advice and will benefit from social connection in a positive environment.

Eligibility criteria: Male aged 18+; BMI 27.5 or above, a Norfolk resident or registered with a Norfolk GP.

MAN v FAT

MAN v FAT Challenge is the UK’s leading digital weight loss programme for men, now offering funded spaces, courtesy of Norfolk County Council. Challengers will receive 14 weeks free, where they will be set weekly challenges to score points based on nutrition, exercise, and building healthy habits. Challengers also have access to weekly zoom calls discussing different weight loss and wellbeing topics, an Online Coach, access to the exclusive MAN v FAT online gym and the opportunity to win medals and awards for weight loss. MAN v FAT Challenge puts men into a virtual team with men from the funded areas across the country.

Eligibility criteria: Male aged 18+ with a BMI 25 or higher and a Norfolk postcode.
