NHS digital weight management programme referral request

Use this service to register your choice to be referred to the NHS digital weight management programme.

You can use this service if you:

• are registered at the surgery
• are over 18 years old
• have a BMI of over 30 or 27.5 if you are BAME
• have diabetes type 1 or 2 and/or hypertension

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
