Please use this form to inform us you are pregnant, to ensure you receive the appropriate antenatal (pregnancy) care.

Following completion of this form please call the community midwives hub on 01603 287239 to book an appointment with a Community Midwife. If you are less than 6 weeks pregnant, please do this when you are between 6 – 8 weeks. If you are more than 8 weeks pregnant, please do this now.

You can refer yourself to either the Norfolk and Norwich or James Paget for your maternity care by completing one of the forms below:
